SUMMARY:"Transition to Adulthood in Health" Listening Event
DESCRIPTION:Manchester University NHS Foundation Trust (MFT) provides a range of services for many children and families with Special Educational Needs and Disabilities. For young people moving from children’s health services into adult health services\, the transition is not always smooth and can sometimes be overwhelming.\n\nThe Designated Clinical Officer for SEND in Manchester\, Jennie Corkwood\, is keen to know what parents’ thoughts are on the health transitions in Manchester so that we can consider what works well and where things need improvement.\n\nThis event is aimed at:\n\n\nyoung people\, parents and carers\, who have been through the process of transition to adult services (over 16 years)\n\n\nyoung people\, parents and carers who are approaching the transition to adult services\n\n\nJennie is keen to hear and understand your thoughts on things like:\n\n\nWhat information you need/want to know about health transitions and when (just before they happen/ as they happen/ at age 14 etc…)\n\nHow could things be improved for you as part of the health transition?\nHave you been able to access the services you need\, when you need them?\n\nWhen have health services been supportive and ‘got it right’?\n\n\nIf you are unable to attend or prefer not to attend this in-person event\, please let us know your thoughts\, or we can arrange another way for you to share your thoughts with Jennie. \n.fusion-button.button-1 {border-radius:2px;}Book Now\n
