If you booked for a HAF SEND Community Offer activity online, please don’t forget to fill in the Attendee Google Form for your family, making sure to register each person that you booked. (e.g. If you booked for 3 people, send over the details for all 3 individuals in the form. There should be at least one adult and at least 1 child with additional needs aged 5-16 years old.)

❗ Completion of the form is essential for entry to events. Participants who aren’t on our register (taken from the forms) are at risk of being turned away.
